If you are an international student, you will need to follow these additional onboarding requirements here as well as the steps below.

1. International Background Packet.pdf

4. Parking Information and Parking Permits and Maps
5. After completing the Student Compliance and Privacy/Security Training (HIPAA).pptx, fill out the Training Acknowledgment.pdf, then email the program.

U.S based Students

UConn Health Specific Requirements

• Application for Credit Waiver, Transfer or Advance Standing.pdf (if applicable)
• IRB Registration and Instructions – email certificate of completion to Program Support by October 15.
• IRB Forms for Human Subject Research

For fillable forms, we suggest downloading and saving the form to your computer. Then, open with acrobat reader to complete and save.

Please return all forms to the program office, MC 6325. A copy will be kept on file in the Program in Applied Public Health Sciences office, and the original forwarded to the Graduate School.
