Onboarding Forms for Incoming Students

Prior to First Class

International Students

If you are an international student, you will need to complete these additional onboarding forms here as well as the steps below.

  1. International Background Packet.pdf
  2. Background Check Form.pdf 
  3. Immunization Requirements and Form.pdf
  4. Parking Information and Parking Permits and Maps
  5. After completing the Student Compliance and Privacy/Security Training (HIPAA).pptx, fill out the Training Acknowledgment.pdf, then email the program.

U.S based Students

UConn Health Specific Requirements

For fillable forms, we suggest downloading and saving the form to your computer. Then, open with acrobat reader to complete and save.

Please return all forms to the program office, MC 6325. A copy will be kept on file in the Program in Applied Public Health Sciences office, and the original forwarded to the Graduate School.

UConn Health Account & Email Access

Step 1: Reset Your Password
  1. Go to UConn Health Password Reset.
  1. Sign in using UCHC\username (example: UCHC\JSMITH).
  2. Enter your initial password:

Format: Capital first letter of your first name + lowercase first letter of your last name + birthdate (YYYYMMDD)

Example: John Smith, born January 1, 2002 → Js20020101

If your birthdate was not provided, use 19000101 instead (example: Js19000101).

Step 2: Create a New Password

When prompted, create a new password that:

  • Is at least 8 characters long
  • Includes uppercase and lowercase letters
  • Includes at least one number
  • Includes at least one symbol
Step 3: Set Up DUO Authentication

After resetting your password, follow the prompts to enroll in DUO Two-Factor Authentication.
